The patient pays an annual membership fee to join, which covers their physical and a set number of hours of the physician’s time. There are family rates and individual rates. The patient and physician sign an agreement that outlines the services covered.
Who can be a patient?
Dr. Fagan accepts patients of all ages but only does preventive care for patients age 5 and older.
Does your practice accept insurance?
Happily, no!
Perhaps the most critical distinction in this model of care centers on a direct, personal relationship between you and your doctor. Dr. Fagan prefers that no insurer interferes with either the quality or the cost of your family’s health care.
As a patient of Hometown Doc, LLC, will I still need health insurance?
Yes. Dr. Fagan recommends that her clients continue to carry a major medical plan to ensure financial help should hospitalization or referral to a specialist be necessary.
If I have Medicare, can I still join Hometown Doc, LLC?
Yes. You need only sign a one-time waiver declaring that neither you nor your doctor will directly bill Medicare for Dr. Fagan's services. Medicare will still cover any laboratory testing, imaging, medications or hospitalizations prescribed by Hometown Doc.
What happens if I need to go to the hospital or see a specialist?
Dr. Fagan seeks to minimize hospitalizations and specialty referrals by providing unprecedented access. However, in the event of hospitalization or referral, she efficiently coordinates and monitors additional medical specialists as required.
How much does it cost?
Our annual fees are less than you might expect! Rates vary based on number of individuals, but fees range from $2000-$5000 per person annually.
